Uterine leiomyomas are extremely rare in girls younger than age 18. Only 19 cases were reported between 1965 and 2014.

This video presents a rare case of a 14-year-old, gravida-0 female with a 9 cm intramural fibroid. Cleveland Clinic gynecologists guide the discussion from presentation to diagnosis and management.

While fibroids are rare in adolescent girls, they should be included as part of the differential diagnosis for pelvic masses. Fibroids appear to behave in adolescents as they do in older women. However, difference in tumor biology must be taken into consideration. An optimal treatment has not been defined, but myomectomy has many clear advantages, especially for preserving fertility for these young women.
